This form is required to verify the<br \/>\nnature and impact of your disability or condition.<\/p>\n<h3>Step 2: Complete the Online Accommodation Request Form<\/h3>\n<p><a href=\"https:\/\/saintpeters-accommodate.symplicity.com\/public_accommodation\/\" target=\"_blank\" rel=\"noopener\">Fill out the Accommodation Request Form<\/a>. When completing the form, you will be asked to upload:<\/p>\n<ul>\n<li>the completed Certification of Health Care Provider Form and<\/li>\n<li>any additional documentation supporting your request.<\/li>\n<\/ul>\n<p>Supporting documentation may include:<\/p>\n<ul>\n<li>Educational or psychological evaluations<\/li>\n<li>IEPs (Individualized Education Plans) or 504 Plans from prior educational<br \/>\nsettings<\/li>\n<li>Letters of Accommodation from previous colleges or universities (for transfer<br \/>\nor graduate students).<\/li>\n<\/ul>\n<p><strong>Please note:<\/strong> All documentation must be within the last five years of the current<br \/>\nacademic year.<\/p>\n<h3>Step 3: Intake Meeting<\/h3>\n<p>After we receive your completed application and documentation, a member of our team<br \/>\nmay request a meeting with you to discuss your accommodations if needed. This<br \/>\nmeeting provides an opportunity to:<\/p>\n<ul>\n<li>Review your submitted materials;<\/li>\n<li>Discuss your needs and experiences;<\/li>\n<li>Determine reasonable and appropriate accommodations.<\/li>\n<\/ul>\n<h3>Step 4: Submit Requests Early<\/h3>\n<p>To help ensure your accommodations are in place at the start of the semester, we<br \/>\nrecommend the following timelines:<\/p>\n<p><strong>Academic Accommodations (New and Semester Requests)<\/strong><\/p>\n<ul>\n<li>Fall Registration: August 1st<\/li>\n<li>Winter Registration: October 1st<\/li>\n<li>Spring Registration: December 1st<\/li>\n<li>Summer Registration: May 1st<\/li>\n<\/ul>\n<p><strong>Housing Accommodations<\/strong><\/p>\n<ul>\n<li>Returning Students: May 1st<\/li>\n<li>New Students: July 15th<\/li>\n<\/ul>\n<p>Late submissions will still be reviewed, but timely requests ensure smoother<br \/>\ncoordination and implementation.<\/p>\n<h3>Need Assistance?<\/h3>\n<p>If you have any questions, need help completing forms, or want to better understand the<br \/>\nprocess, please do not hesitate to reach out to <a href=\"mailto:accommodations@saintpeters.edu\">accommodations@saintpeters.edu<\/a>.
